A workshop series focused on Youth with Special Health Care Needs.

The Transitioning Together Workshop series, hosted monthly by the NCED Family Navigation Network, supports families with youth who have special health care needs and/or disabilities. Each session begins with a guest speaker discussing a specific topic related to the transition from childhood to adulthood, providing valuable insights and information.

The workshops are designed to be interactive, featuring hands-on exercises that help participants apply what they've learned. Led by Transition Coordinator, Maria Barber, these sessions offer practical tools and guidance to assist families in navigating this critical life stage.
