Service-Learning and internship DocuSign forms
Each document contains:
- Student Learning Agreement
- Indemnification waiver
- Covid-19 waiver
Please provide student with the appropriate link based on the type of internship or service-learning activity.
Students will be asked to enter their name and email address so that they will receive a copy of the signed form and are asked to enter the name and email address of their instructor/internship advisor so that they will also receive a copy of the students signed waiver.
